What do you need to do?

Please complete all the relevant forms for your claim and return to GPO Box 5107, Sydney NSW 2001.

If you have any questions about how to complete your application or any general queries, please call us on 13 12 87 weekdays between 8.30 and 6.30pm (AEST) to reach one of our friendly contact centre staff.

If you wish to complete a rollover of your superannuation monies or make a lump sum withdrawal please call 13 12 87 to speak with a specialised consultant. These forms are not to be used and the consultant will advise you of your next steps.

Please contact 13 12 87 if the insurance through the superannuation account is with an insurer other than OnePath Life Limited – you can find these details on your last annual statement.

Death with insurance

Going through the loss of a loved one is very difficult and we are here to help you through this process. You may be eligible to the late members superannuation monies, you may also be entitled to insurance proceeds if the late member had Death Insurance cover through their superanuation account at the time of their death.

If you are unsure whether the member held insurance through their superannuation account, please complete the documentation listed below

What do you need to do?

In order for the Trustee to assess your claim, please complete and return to our office the following:

You will also need to provide a:
  • Certified copy of the full Death Certificate (not an extract)
  • Certified copy of the late member’s Will (if applicable)
  • Certified copy of the deceased member’s identification
  • Certified copy of Grant of Probate or Letters of Administration (only if already obtained)
  • A listing of the estate’s assets and liabilities or a written statement as to the estate’s solvency
  • Certified copy of the identification of the nominated beneficiary/ies (Only if there is a nominated beneficiary)
  • Certified copy of the identification of the person claiming (Only if no nominated beneficiary or if nominee is estate)
Upon receipt of the requirements we will assess the appropriate beneficiary / beneficiaries to receive the late member’s superannuation monies. We will also forward your claim to the insurer who will assess if the insurance claim can be admitted, and will notify you if further information is required.

Reversionary: A 'reversionary beneficiary' is the person who will continue to receive a member’s pension when the member dies. The reversionary beneficiary is nominated by the member upon the commencement of a pension.

  • Original certified identification of each beneficiary/claimant/executor; e.g. current driver’s licence or passport (current or expired no more than two years)
If the deceased member has a valid Estate please also provide the following:
  • An Original certified copy of the Will
  • An Original certified copy of Probate or Letters of Administration (if applicable)
  • An inventory of the Estate's assets and liabilities or a letter from you/your office confirming the Estate's solvency
  • Tax file number of the deceased or the tax file number of the Estate
